No child may go on a field trip unless a "permission slip" is returned to the school signed by the parent or guardian. Permission for a student to attend a field trip may not be given verbally, in person, or over the phone. All drivers for field trips must have a completed form on file in the CKACS office, along with a photocopy of their "driver's license" and "insurance policy."
